However, a serious problem arises when we allow the act of \u2018performing\u2019 safety to mask what is really going on behind the scenes. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Over the years we have been conditioned to see the absence of workplace incidents as a sign that all is well with our safety management system. That the field of green that appears on our audit dashboards month after month \u2013 even year after year \u2013 means that risk is being managed. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">However, a reliance on these metrics, especially if it is a select few, often ignores the wider context of the business. It developed from the work of sociologist <a href=\"https:\/\/psychsafety.com\/normalisation-of-deviance\/\" target=\"_blank\" rel=\"noreferrer noopener\">Diane Vaughan<\/a>, who coined the \u2018normalisation of deviance\u2019 term during her research on the Space Shuttle Challenger disaster.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Safety drift is usually driven by factors such as workplace complacency and operational success. Complacency in the idea that nothing (e.g. a serious incident) will ever happen because nothing ever has, and operational success in that visible success often hides the series of small safety compromises that build up over time. <\/p>\n\n\n\n<div style=\"height:30px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h3 class=\"wp-block-heading has-medium-font-size\">\u201cAccidents hide in the green\u201d: when good numbers are the warning sign<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Why should audits and safety dashboards that return uniformly positive or \u2018green\u2019 results give us pause? There are some <a href=\"https:\/\/en.wikipedia.org\/wiki\/Texas_City_refinery_explosion\" data-type=\"link\" data-id=\"https:\/\/en.wikipedia.org\/wiki\/Texas_City_refinery_explosion\" target=\"_blank\" rel=\"noreferrer noopener\">individual case studies<\/a> that suggest that incidents and fatalities can occur after long spells of apparently incident-free work, particularly when safety performance is only judged by select indicators. However, due to the practical and ethical challenges in studying this as part of controlled trials, empirical evidence is hard to come by.  <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Instead, prominent safety scientist and researcher <a href=\"https:\/\/media.licdn.com\/dms\/document\/media\/v2\/D561FAQEFCHW8IHE6Hw\/feedshare-document-pdf-analyzed\/B56Zm6kFTeIsAY-\/0\/1759771647011?e=1788393600&amp;v=beta&amp;t=R9Wfm0bnswC13Sq1EgrikIwAa2UXfzC4d04AQKgDhl0\" data-type=\"link\" data-id=\"https:\/\/media.licdn.com\/dms\/document\/media\/v2\/D561FAQEFCHW8IHE6Hw\/feedshare-document-pdf-analyzed\/B56Zm6kFTeIsAY-\/0\/1759771647011?e=1788393600&amp;v=beta&amp;t=R9Wfm0bnswC13Sq1EgrikIwAa2UXfzC4d04AQKgDhl0\" target=\"_blank\" rel=\"noreferrer noopener\">Sidney Dekker<\/a> suggests this concept can be framed around the idea of \u2018operational success\u2019. That is, \u201ca growing incident potential produced by the very means of creating operational success\u201d. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For example, if a business\u2019 mark of success is keeping the cost of production down while maintaining (and exceeding) production quotas, it&#8217;s inevitable that there will be a cost to workplace safety. Safety budgets are cut, headcount is reduced, cheaper PPE is sourced, and workers are under increased pressure to do more faster and with less. If keeping lagging indicators like the number of slips or trips down is a mark of good safety performance, it doesn\u2019t mean that risk is being controlled elsewhere. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It\u2019s important to remember that your dashboard is only as good as the data you put into it. Are you recording your near-misses and other safety observations? <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Another factor to consider is the potential effect of low reporting among employees. Your numbers may say that slips and trip incidents are down, but what if this is due to underreporting? Are workers subtly discouraged from raising concerns? The strength of your <a href=\"https:\/\/www.ecoonline.com\/en-nz\/blog\/how-do-i-evaluate-my-organisations-health-and-safety-culture-and-supporting-technology\/\" data-type=\"link\" data-id=\"https:\/\/www.ecoonline.com\/en-nz\/blog\/how-do-i-evaluate-my-organisations-health-and-safety-culture-and-supporting-technology\/\">safety culture<\/a> plays a key role here.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">There is also the simple fact that there are many aspects to good workplace safety that are not quantifiable with an attractive metric. Everything looks safe until it isn&#8217;t because accidents don&#8217;t hide in the red. They hide in the green. The performance of compliance <\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Safety theatre refers to the performance of work when subject to some form of surveillance, for example when a supervisor is present. It was <a href=\"https:\/\/media.licdn.com\/dms\/document\/media\/v2\/D561FAQEFCHW8IHE6Hw\/feedshare-document-pdf-analyzed\/B56Zm6kFTeIsAY-\/0\/1759771647011?e=1788393600&amp;v=beta&amp;t=R9Wfm0bnswC13Sq1EgrikIwAa2UXfzC4d04AQKgDhl0\" data-type=\"link\" data-id=\"https:\/\/media.licdn.com\/dms\/document\/media\/v2\/D561FAQEFCHW8IHE6Hw\/feedshare-document-pdf-analyzed\/B56Zm6kFTeIsAY-\/0\/1759771647011?e=1788393600&amp;v=beta&amp;t=R9Wfm0bnswC13Sq1EgrikIwAa2UXfzC4d04AQKgDhl0\">originated by Dekker<\/a> who used it to describe the image of work as imagined vs. work as it is done out of view. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Safety theatre highlights the distance between the planners and designers of work, who are assumed to be correct, and the people who carry out the work, who are assumed to be at fault if they do not follow procedure to the letter. How the work was designed to be done \u2018officially\u2019 may not reflect reality on the ground. <\/p>\n\n\n\n<div style=\"height:30px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h3 class=\"wp-block-heading has-medium-font-size\">Work-as-imagined during the inspection, work-as-done after it<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">For certain tasks there is a commonly accepted \u2018right\u2019 sequence of events. Take a lockout procedure \u2013 the isolation is verified, the permit is obtained, and a supervisor reviews. The work is now completed according to procedure. On the day of a safety inspection, this is what the auditor sees and records. There is no reason to think workers would choose to do things the \u2018wrong\u2019 way.    <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Real-life circumstances often differ. The next day there may be a production emergency and the supervisor may not be present to review the permit. With the equipment needed \u2018urgently\u2019, established workarounds are used to complete work on time. In this way, the \u2018dangerous\u2019 option is reinforced until an accident occurs.   <\/p>\n\n\n\n<div style=\"height:30px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h3 class=\"wp-block-heading has-medium-font-size\">What is performative safety? If this doesn\u2019t happen, they are at risk of falling into a compliance culture \u2013 where surface-level adherence to rules, checklists and passing inspections becomes the ultimate goal of workplace safety.<\/p>\n\n\n\n<div style=\"height:30px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h3 class=\"wp-block-heading has-medium-font-size\">How tick-box safety creates a false sense of control<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Human beings <a href=\"https:\/\/www.psychologytoday.com\/ie\/blog\/the-narrative-nurse-practitioner\/202512\/the-psychological-benefits-of-lists\" data-type=\"link\" data-id=\"https:\/\/www.psychologytoday.com\/ie\/blog\/the-narrative-nurse-practitioner\/202512\/the-psychological-benefits-of-lists\" target=\"_blank\" rel=\"noreferrer noopener\">like to make lists<\/a>. When we check something off a list or mark a task as completed, we experience a dopamine hit. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">However, in workplace safety, relying on so-called \u2018<a href=\"https:\/\/consultmesh.co.uk\/insights\/health-and-safety-is-not-a-tick-box-exercise\/\" data-type=\"link\" data-id=\"https:\/\/consultmesh.co.uk\/insights\/health-and-safety-is-not-a-tick-box-exercise\/\" target=\"_blank\" rel=\"noreferrer noopener\">tick-box safety<\/a>\u2019 often devolves into a paperwork exercise. A high-risk job requires a risk assessment and on paper, one has been completed and signed off on. <strong>Mark as complete<\/strong>. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">On closer inspection we see that the risk assessment is a generic template that does not pertain to the specific task being done. The risk to the worker has not <em>actually<\/em> been controlled. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Another common example is in <a href=\"https:\/\/www.ecoonline.com\/en-nz\/blog\/workplace-safety-training-dont-become-a-statistic\/\" data-type=\"link\" data-id=\"https:\/\/www.ecoonline.com\/en-nz\/blog\/workplace-safety-training-dont-become-a-statistic\/\">workplace safety training<\/a>. Extensive training records can show that employees have sat through all the required training and completed every module. <strong>Mark as complete<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This does not prove competence in performing a task. The problem with chasing zero <\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0022437524001178#s0005\" data-type=\"link\" data-id=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0022437524001178#s0005\" target=\"_blank\" rel=\"noreferrer noopener\">Zero harm<\/a> or injury has been a popular buzzphrase among health and safety circles for several years. <a href=\"https:\/\/www.ioshmagazine.com\/2023\/02\/27\/zero-harm-concept\" data-type=\"link\" data-id=\"https:\/\/www.ioshmagazine.com\/2023\/02\/27\/zero-harm-concept\" target=\"_blank\" rel=\"noreferrer noopener\">It can be defined<\/a> as the pursuit of a \u2018no harm or fatalities at work\u2019 policy.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It has been incorporated by large global businesses. Safety programmes incorporating zero harm are often based on the belief that fewer reported incidents mean greater safety. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Zero harm has its detractors, however. It has been criticised as being an aspirational ambition as opposed to an achievable safety goal. <\/p>\n\n\n\n<div style=\"height:30px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<div style=\"height:30px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h3 class=\"wp-block-heading has-medium-font-size\">Is zero incidents a good measure of safety? What the research shows <\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">There is little research on the effectiveness of zero harm\/incidents. The <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0022437524001178#s0010\" data-type=\"link\" data-id=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0022437524001178#s0010\" target=\"_blank\" rel=\"noreferrer noopener\">studies we do have<\/a> suggest no evidence of the benefits of \u2018zero\u2019 as a policy in practice. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/media.licdn.com\/dms\/document\/media\/v2\/D561FAQEFCHW8IHE6Hw\/feedshare-document-pdf-analyzed\/B56Zm6kFTeIsAY-\/0\/1759771647011?e=1788393600&amp;v=beta&amp;t=R9Wfm0bnswC13Sq1EgrikIwAa2UXfzC4d04AQKgDhl0\" data-type=\"link\" data-id=\"https:\/\/media.licdn.com\/dms\/document\/media\/v2\/D561FAQEFCHW8IHE6Hw\/feedshare-document-pdf-analyzed\/B56Zm6kFTeIsAY-\/0\/1759771647011?e=1788393600&amp;v=beta&amp;t=R9Wfm0bnswC13Sq1EgrikIwAa2UXfzC4d04AQKgDhl0\" target=\"_blank\" rel=\"noreferrer noopener\">Dekker suggests<\/a> that it may even have negative effects. If zero is the safety policy, it can lead to a lack of willingness in the organisation to accept bad news. Workers may be discouraged, directly or otherwise, to report non-conformances that may affect the \u20180\u2019 score. It is similar to how signs such as \u2018X days since last incident\u2019 work. The higher the number on the sign, the higher the threshold to \u2018break\u2019 the streak. That there are often financial bonuses tied to these figures further muddies the picture.     <\/p>\n\n\n\n<div style=\"height:30px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h3 class=\"wp-block-heading has-medium-font-size\">The fundamental regulator paradox: how zero blinds you to risk<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Another problem presented by zero is seen in the fundamental regulator paradox. If the data is reduced to zero, then there is no way for safety managers to make improvements or spot risk. <\/p>\n\n\n\n<figure class=\"wp-block-pullquote\"><blockquote><p>\u201c\u2026the focus is on reducing simple negative data points (incidents), which are often ignored, repressed, underreported or case-managed into statistical oblivion. How can you address and stop safety drift from occurring? <\/p>\n\n\n\n<div style=\"height:30px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h3 class=\"wp-block-heading has-medium-font-size\">Make reporting easy so bad news reaches you<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">One of the marks of a strong safety culture is the active engagement with employees. You need to have a way for safety <a href=\"https:\/\/www.ecoonline.com\/en-nz\/blog\/how-incident-reporting-drives-workplace-productivity-safety\/\" data-type=\"link\" data-id=\"https:\/\/www.ecoonline.com\/en-nz\/blog\/how-incident-reporting-drives-workplace-productivity-safety\/\">incidents<\/a>, near-misses, observations and feedback to be reported easily by your workers. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This \u2018bad news\u2019 is how you will identify areas of risk. Encouraging reporting and making quick, visible remedial efforts are an excellent way to build trust among your workers in your safety culture. Workers can see that their reporting has an effect, and they will be more likely to report in the future.  <\/p>\n\n\n\n<div style=\"height:30px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h3 class=\"wp-block-heading has-medium-font-size\">Track leading indicators, not just a clean scoreboard<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">We have discussed how relying solely on lagging indicators is a sub-optimal way of judging safety performance and identifying safety drift.
