Everyone inside heard it. People on the surface heard it. At almost exactly the same time, Titan’s sensors recorded a burst of acoustic activity and a sudden change in the strain data from its carbon fiber hull. Stockton Rush looked at that data, and according to the NTSB, his response was, let’s dive again and see what happens. That was dive eighty. Titan imploded on dive eighty eight. And five people died. And what’s worse, that bang wasn’t the first warning. For years, people inside Ocean Gate and experts outside the company had raised serious concerns about how Titan was being designed, tested, and operated. Some tried to stop it, and one actually succeeded in getting an earlier hull taken out of service. But the program kept moving forward. Quick thing before we go back. If you like real stories about disasters, the decisions behind them, and the safety lessons that came afterward, hit like, subscribe, and turn on all notifications so you don’t miss another story. Alright, back to twenty eighteen. David Lockridge was Ocean Gate’s director of marine operations. Stockton Rush asked him to inspect Titan’s first full scale pressure vessel. Lockridge came back with serious concerns. Titan’s center section was carbon fiber, which can develop internal damage that isn’t obvious from the outside. Lockridge reported concerns including delamination and porosity and wanted nondestructive testing to look for hidden flaws inside the material. Ocean Gate planned to rely heavily on acoustic monitoring instead. Sensors would listen to the hull while it was under pressure. Lockridge thought that warning could come far too late, obviously. His later legal complaint argued that acoustic monitoring might detect failure only milliseconds before an implosion. Enough time to know something’s wrong, not enough time to get back to the surface. He also raised concerns about other parts of Titan’s design and eventually refused to approve manned testing without more testing of the hull. He told Lockridge he had fundamental concerns about Titan’s safety. I am highlighting my safety concerns, which, as an employee of the company, I’ve been here for nearly three years now, Stockton, I’ve seen the way it works. I am addressing what I view as safety concerns, concerns that I have mentioned verbally Correct. Which have been dismissed by everybody. I have I no. I’ve listened to them, and I have given you my response to them. It is. And you think my response is inadequate. Correct. Okay. So that’s the impact for And four days later, Lockridge was fired. He filed a whistleblower complaint with Osha. Ocean Gate sued him. The case was eventually settled, and Titan development continued. Then just weeks later, Ocean Gate got another warning. On March twenty seventh twenty eighteen, experts from the Marine Technology Society’s Committee on Manned Underwater Vehicles sent a letter directly to Stockton Rush. The concern was that Ocean Gate was developing an experimental passenger carrying submersible without the independent review normally used in the industry. Their letter warned that Ocean Gate’s approach could lead to outcomes ranging from minor to catastrophic, five years before the implosion. They urged Rush to let an outside organization like DNV or ABS review and witness the testing. Ocean Gate didn’t. The NTSB later documented Rush describing classification as too expensive and not doable. Titan was never classed. Then in twenty nineteen, Rush was testing Titan’s original pressure hull in the Bahamas. Submersible operator Carl Stanley went with him on one of those deep dives. Stanley heard cracking noises coming from the hull and warned Rush that the sounds could indicate a defect getting worse under pressure. He urged Rush to slow down before taking paying passengers to Titanic. Rush disagreed. Later that year, an Ocean Gate employee found a hairline crack in Titan’s first pressure vessel. The crack originated from an internal delamination. This time, somebody stopped the program. Ocean Gate’s first director of engineering refused to approve that hull for planned Titanic expedition. According to the NTSB, that engineer was terminated shortly afterward, but the expedition was canceled. The hull was tested further and eventually condemned. So the warning worked. A crack was found. An engineer refused to sign off. The hull was retired. But Ocean Gate did not abandon carbon fiber design. They built another one. They changed the manufacturing process, tested scale models, learned from failures, and built a second full scale pressure vessel, the hull that would eventually implode. And Titan started successfully reaching Titanic. By the end of twenty twenty two, it had reached the wreck site thirteen times. Every successful trip made the design look safer. But there was one question Ocean Gate had never properly answered. How many deep dives could this specific hull safely survive? The NTSB later found Ocean Gate never performed proper life cycle evaluation on the final pressure vessel. Every dive loaded that carbon fiber structure with enormous pressure, then released it. Ocean Gate had calculations for how strong it should be, but investigators later found manufacturing anomalies in the actual structure. They never established the real safe lifespan of that specific hull. Instead, they relied heavily on Titan’s monitoring system. Titan had eight acoustic sensors listening to the hull and sixteen strain gauges measuring how it physically changed under pressure. There were warning thresholds. According to Ocean Gate’s final director of engineering, thirty qualifying acoustic events during a dive triggered yellow. Fifty triggered red, and red meant abort the dive. That sounds reassuring, but investigators later found Ocean Gate had never demonstrated that system could warn the crew early enough to save them. From titanic depth, getting back to the surface could take hours. That capability was never proven. Then came July fifteenth twenty twenty two, dive eighty. Titan had visited Titanic and was returning toward the surface, and suddenly, bang, everyone inside heard it. People at the surface heard it. Passenger Fred Hagen later said you’d have to be brain dead not to be concerned. People were told Titan had shifted in its metal launch cradle. The sound came from the vehicle moving against the frame, not the pressure hull. Except Titan’s sensors had recorded something at almost exactly the same moment, a burst of acoustic activity and a sudden change in multiple strain gauges. Rush reviewed that data with Ocean Gate’s engineering director. According to the NTSB, he said, let’s dive again and see what happens. So they did. Dive eighty one reached titanic depth and returned. Dive eighty two did the same. Once those dives worked, the explanation for the bang probably felt even more convincing. But surviving a risk doesn’t prove the risk was safe. After the bang on dive eighty, the hull was behaving differently. The NTSB concluded Titan likely suffered one or more delaminations after that dive. Layers inside the carbon fiber had begun separating, and investigators found evidence of additional damage after dive eighty two. And when pieces of Titan were recovered, investigators found physical evidence of delamination in the wreckage. The NTSB’s conclusion was blunt. Titan should have been immediately removed from service after dive eighty, and it wasn’t. By June twenty twenty three, it was heading back to Titanic. June eighteenth, five people climb inside. Stockton Rush, Paul Henry Norgelette, Hamish Harding, Shazada Daywood, and his nineteen year old son, Solomon. Ocean Gate personnel close Titan’s forward titanium dome. Eighteen bolts secure it from the outside. At around nine in the morning, Titan begins descending. Titanic is roughly twelve thousand five hundred feet below the surface, and at that depth, the pressure is around fifty five hundred pounds per square inch. At ten forty seven, it still hasn’t reached the Titanic. Then a message reaches the surface saying Titan has dropped both of its drop weights. Drop two weights. Titan dropped two weights. Canada’s investigators later noted those weights were normally released closer to the seabed. Their early release suggests the crew may have been trying to slow the descent. The message appeared on the support team’s computer at ten forty seven twenty six nine zero three. Canada’s Transportation Safety Board calculated that Titan’s catastrophic failure occurred five point three nine seven seconds later. Titan never reached Titanic. The pressure hull failed, the submersible imploded, all five people died. The people on the surface didn’t know that yet. Titan had lost communications before, so losing contact didn’t initially mean catastrophe. Then came four days of searching and the countdown of Titan’s theoretical oxygen supply. Hour, maybe less than twelve hours left. If they stay alive keep them busy, so you’re the time limit. Time left. But the disaster had already happened. On June twenty second, an ROV found the wreckage near Titanic. Then the investigations began. The NTSB concluded Ocean Gate’s engineering process was inadequate. The final hull contained manufacturing anomalies. Its actual strength and durability had not been properly established. There was no proper life cycle evaluation. The monitoring data was analyzed incorrectly. Damage after dive eighty was missed. Additional damage accumulated, and eventually, the weakened pressure vessel could no longer withstand the pressure. The hull deteriorated progressively through repeated pressure cycles until the structure failed. But the Coast Guard also looked at Ocean Gate itself. Multiple former employees told investigators that concerns about Titan’s design were ignored. One former engineering director said Rush wouldn’t hire anybody that disagreed with him. He ran the company, was deeply involved in the engineering, and personally piloted Titan. The coast guard called the loss of Titan and the five people aboard preventable. And if Rush had survived, the marine board said it would have recommended referring evidence of potential criminal offense to the Department of Justice. So let’s recap. Lockridge raised serious concerns and was fired. Industry experts warned Rush directly. Carl Stanley warned him after hearing cracking noises. An engineer refused to approve the first cracked hull, and that hull was condemned. Then, after the bang on dive eighty, Titan’s own sensor data showed changes investigators now believe were evidence of damage. Rush reviewed that information and chose to continue. After the disaster, investigators issued dozens of safety recommendations, calling for stronger oversight of human occupied submersibles, better engineering verification, clearer operating requirements, improved whistleblower coordination, and stronger international standards. Some are still only recommendations and not completed regulations, but they all target the same gap Titan exposed, a passenger carrying experimental vessel operating without the independent oversight normally expected in an industry where one structural failure can kill everyone on board. And the ocean itself did nothing unusual. The pressure wasn’t unexpectedly high. The Titanic wasn’t deeper than anyone thought. Years of warnings came before the final dive. After the bang, after the sensors changed, after looking at the data, Stockton Rush said, let’s dive again and see what happens. And for a while, nothing happened until it did. If you got something out of this story, hit like, subscribe, and turn on all notifications so you don’t miss the next one. The team stops work in the area around the scaffold platform with the missing toe board and keeps people clear of the drop zone. A missing barrier can expose people below to falling materials.

1. The inspection creates a finding. The inspector records the platform, task, and condition. The site supervisor and EHS lead can review the same finding and arrange an immediate check of nearby working platforms.

2. The team investigates. Workers explain that they move materials through this area several times a day. The team checks whether the barrier has loosened, whether anyone altered the scaffold, and whether the planned material route still works.

3. The team chooses a control. A competent scaffolder restores the barrier and assesses the access arrangement. The team changes the material route if the task repeatedly puts the barrier at risk. NIOSH’s hierarchy of controls helps teams consider controls that prevent exposure at the source, rather than relying only on reminders.

4. A later inspection tests the change. Inspectors check the platform while crews move materials. They verify that the barrier stays in place and that people below remain protected. If the control fails, they reopen the action.

5. The risk assessment carries the lesson forward. The EHS team reviews the work-at-height and dropped-object risks, updates the assessment and inspection prompts where needed, and checks similar tasks on other parts of the project.

An inspection started the cycle. Ask whether the control addresses the cause, works during normal operations, and appears in the risk assessment and inspection plan where needed.

A repeat missing barrier may show that one repair left a problem with the material route untouched. Connecting the records helps the team review the assessment, change the control, and check similar work elsewhere. The UK HSE’s investigation guidance links investigation findings to risk assessment review. Ireland’s HSA also advises employers to consider incident and inspection records during safety reviews.

How do audits and inspections help prevent repeat incidents?
Inspections check conditions and controls during normal work. Audits examine whether the wider safety process works as intended. When teams connect those findings to incident actions and risk assessments, they can confirm a fix and spot recurring gaps. Closing a task in software alone does not prove that the team has controlled the hazard.

What role does near-miss reporting play in incident prevention?
Near-miss reports show where harm could have happened. Teams can investigate the conditions, choose a control, and verify it before someone gets hurt. Make reporting simple and respond visibly to workers. Speak with workers who know the task. Then choose actions that address what you found and check whether those actions work in practice. CCOHS guidance describes this follow-through as part of an investigation.

Can EHS software prevent incidents on its own?
No. EHS software can help teams see hazards, assign work, track overdue actions, and identify repeat patterns.
