Introduction

Assessing safety performance can be a notoriously tricky task. While some metrics are self-evident – the number of incidents and injuries, the number of audits completed, the number of slips, trips and falls – they don’t provide a complete picture of safety performance in an organisation. How can you truly measure the effectiveness of your safety programme, and how can you use this knowledge to spot the early signs of safety drift?

What are safety KPIs?

Safety KPIs (key performance indicators) are the metrics by which an organisations safety performance is measured. The chosen safety KPI needs to provide quantitative or semi-quantitative, relevant data that reflects the safety and health issues in the organisation.

In general, safety KPIs should cover information on existing or emerging risks, exposures, and hazards. They should also cover preventative activities to reduce remaining hazards and risks.

Some aspects of safety performance are hard to measure with an objective metric or single figure. These include elements like worker satisfaction or attitudes towards safety.

Safety KPI’s are typically divided into ‘lagging’ (reactive) or ‘leading’ (proactive) indicators. We will take a look at both of these terms later in this blog.

Why the right safety KPIs surface drift before it becomes an incident

We have covered the concept of safety drift extensively in some of our other blogs, but to give it a quick definition:

“Safety drift is the accumulated gap between agreed and actual safety-related controls across an organisation.”

Utilising the right KPIs to measure safety will help you to recognise the signs of safety drift in your organisation early, ensuring that this gap doesn’t provide room for incidents to occur.

There are an innumerable amount of safety KPIs that you could track. It is vital that you are tracking the right ones for your business and industry and not just adding clutter to a safety dashboard for the sake of it. According to EU-OSHA, it is only useful to measure a performance indicator if the KPI can be used for OSH (occupational safety and health) decision making.

The two types of safety KPI every EHS team needs

Most safety KPIs are divided into two groups:

  • Lagging indicators – reflect safety performance in the past, i.e. the information ‘lags’
  • Leading indicators – provide feedback on performance before a safety problem arises, the idea being that steps can be taken to prevent it.

Lagging indicators are by far the most popular KPIs used in organisations. This tends to be because they are easier to measure, quantify, and compare. However, the best practice approach is to use a combination of leading AND lagging indicators to get a full picture of safety performance.

Let’s take a closer look at leading and lagging indicators.


Lagging indicators in safety: measuring what already happened

Lagging indicators form some of the most well-known and recognisable metrics by which safety performance is measured. Historically, prominent safety models such as Heinrichs Triangle have overcounted lagging indicators in suggesting a relationship between minor and major incidents, though it has been critiqued in recent decades.

Below is a list of some of the most common lagging indicators:

TRIR, DART & LTIFR: the core lagging metrics

TRIR – Total recordable incident rate

TRIR measures the rate of work-related injuries and illnesses. It is the number of incidents which have occurred on-site, usually within a year, according to a certain number of hours worked by 100 employees. While TRIR is used by many businesses globally, it is closely associated with the Occupational Safety and Health Administration (OSHA) in the USA.

The TRIR calculation is: (N/EH) x 200,000, where N = number of injuries and illnesses; EH = total hours worked by all employees during the calendar year; and 200,000 = base for 100 equivalent full-time workers (working 40 hours per week, 50 weeks per year).

DART

The DART rate (Days away, Restricted or Transferred) measures the number of workplace injuries and illnesses resulting in lost workdays, job restrictions or permanent transfers per 100 full-time workers. There is higher emphasis on severity than in TRIR.

DART too has a strong association with OSHA in the USA. It is calculated in the following way: (N/ EH) x (200,000) where N is the number of cases involving days away and/or restricted work activity, and/or job transfer; EH is the total number of hours worked by all employees during the calendar year; and 200,000 is the base number of hours worked for 100 full-time equivalent employees.

LTIFR

LTIFR (Lost time injury frequency rate) is the number of lost time injuries that occurred during the reporting period. Most companies choose to calculate LTIFR per 1 million man-hours worked. LTIFR is commonly used internationally.

It is calculated as follows: LTIFR = (N / EH) × 1,000,000, where N is the number of lost-time injuries; EH is the total number of hours worked by all employees during the reporting period; and 1,000,000 is the base number of hours worked by 500 full-time equivalent employees in a year.)

Why a green scoreboard can still hide rising risk

As we explored in our blog ‘Safety Theatre: How safety drift hides behind a green dashboard‘, safety dashboards and scoreboards that give us uniformly positive or ‘green’ results can hide the true extent of risk in an organisation. There are some individual case studies that suggest that incidents and fatalities can occur after long spells of apparently incident-free work, particularly when safety performance is only judged by select indicators.

Your dashboard may not tell you the full story. Take a lagging indicator like slips and trips. Your numbers may say that slips and trip incidents are down, but what if this is due to underreporting? Are workers subtly discouraged from raising concerns? The strength of your safety culture plays a key role here.

Leading indicators in safety: measuring prevention you control now

Leading indicators have a predictive value and can be used to improve safety management in general. They allow for intervention before the health and safety of employees is affected. According to a US OSHA publication, leading indicators:

“…are proactive, preventive, and predictive measures that provide information about the effective performance of your safety and health activities.

They measure events leading up to injuries, illnesses, and other incidents and reveal potential problems in your safety and health program.”

Near-miss reporting rate, safety observations, corrective-action closure and training completion

Near-miss reporting rate

A near-miss is an unplanned event that did not result in injury, illness, damage or loss – but had the potential to do so. Your near-miss reporting rate is the measurement of how often workers report these events.

If used as a leading indicator, a near-miss reporting rate can provide early insight into emerging risks. It can also act as a barometer for the strength of your reporting culture in general.

An example from the manufacturing industry could be a forklift moving a pallet through a production area. Without warning, a pedestrian steps out from behind a piece of machinery. The forklift operator is able to break in time and avoid colliding with the pedestrian.

While there is no injury, an event like this is reported as a near-miss. It reveals several hazards, including:

  • Limited visibility
  • Poor traffic management (pedestrians and vehicles interacting)
  • Possible inadequate signage

It highlighted a weakness before an incident took place, presenting an opportunity to revise safety controls.

Corrective-actions closure rate

A corrective action is a measure taken to eliminate the cause of a hazard and/or reduce the chance of it happening again. The corrective-action closure rate is a useful leading indicator as it measures the organisations response to identified risks.

Keep in mind that just using the number of corrective actions closed does not tell us much about safety performance. A better measurement would be the time it takes for a corrective action to be closed (particularly a high-risk one). Recurring actions are also useful to track – especially if the same problem arises.

To continue with our manufacturing example, a high-priority corrective action would be to remove machinery that blocks visibility for forklift operators.

Training completion

Tracking training completion is another useful leading indicator. However, it is vital that completion is not equated with competence. Simply having a 100% training completion rate tells us nothing about the quality of training, whether it is understood and retained by the workers, and whether it has been successfully applied to tasks.

For training leading indicators that are truly meaningful, consider some of the following:

  • The percentage of mandatory training completed on time
  • Percentage of safety-critical roles with competency certification
  • Practical competency assessments
  • Percentage of current training certifications
  • Percentage of workers whose training is at risk of becoming outdated

Safety observation rate

Safety observations can also be a useful leading indicator to track. Unlike near-misses, safety observations do not need to wait for an event to happen. They are proactive and give employers visibility into how work is actually being conducted. They take note of hazardous behaviours, general non-conformance and risk. They can provide a useful early warning that risk is increasing.

Keep in mind that the real value of tracking this as a leading indicator would be found in measuring the quality of observations AND what happens afterwards. An example would be the proportion that identify meaningful hazards, recurring issues or ineffective controls and how quickly these findings lead to corrective action.

What makes a leading indicator actually predictive?

A leading indicator can only be considered predictive when there is evidence that it is linked with future safety outcomes.

Here are 5 criteria you can use to assess if a leading indicator is truly predictive:

  1. There is a supported link to future safety outcomes
  2. It measures something that happens before the outcome
  3. It responds to changes in risk
  4. It is actionable
  5. The predictive relationship is continually reviewed and tested

How the two indicators work together to reveal drift

It would be a mistake to simply say that all lagging indicators = bad and all leading indicators = good. Standards such as ISO 45001, Clause 9.1 (Monitoring, Measurement, Analysis & Evaluation) require the tracking of both leading and lagging indicators on a defined cadence.

Recent years have seen the development of monitoring methods that do not fit a leading or lagging designation, such as HECA (High-energy control assessments) as a proposed TRIR alternative.

Leading and lagging indicators can work together to produce a better overall picture of safety performance, as well as help organisations spot safety drift.

A basic example would start with examining your LTIFR. It’s falling, which on the surface is good news. However, as you are also tracking near-miss reporting as a leading indicator, you can see that the number of reports has noticeably dropped. You also notice that high-priority corrective actions take a long time to close. You can now start examining the reasons behind the lack of reporting and the delay in corrective actions.

In this way, you can identify weakening controls and stop the drift.

So, what safety KPIs should you have on your dashboard that will let you spot drift and close the gap?

Which EHS KPIs to track

Remember there isn’t a one size fits all approach to safety KPIs. They can vary by industry, organisational context, and legal requirement.

Some useful KPI’s that are a mix of leading and lagging indicators can include:

  • LTIFR
  • TRIR/ recordable injury rate
  • Corrective actions closed on time
  • Near-miss and nice catch reporting rate
  • Percentage of mandatory safety trainings completed on time
  • Worker safety participation (safety committees, surveys etc.)
  • Training completion tied to performance improvements
  • Audits completed (particularly repeated audit findings)

They’re essential for benchmarking and compliance, but they’re reactive – they measure harm that has already occurred, and can encourage underreporting if over-emphasised.

How do you calculate TRIR?

TRIR (Total Recordable Incident Rate) = (number of recordable incidents × 200,000) ÷ total hours worked. The 200,000 factor represents 100 full-time workers over a year. For example, 8 recordable incidents across 490,000 hours worked gives a TRIR of 3.27. Always compare against your industry benchmark, as “good” varies by sector.

Which safety KPIs best detect safety drift?

Leading indicators detect drift earlies – especially near-miss reporting rate, corrective-action closure time and audit or inspection scores trending downward. A falling near-miss report count often signals underreporting rather than a safer site. Watching these signals alongside lagging outcomes reveals the widening gap between policy and practice before it becomes an incident.
